Enhanced Care Management (ECM): Our Enhanced Care Management (ECM) services in Santa Clara County are designed to provide personalized care management for patients with specific insurance coverage, including: Santa Clara Family Health Plan Anthem Blue Cross (Santa Clara County) Valley Health Plan Our ECM program offers tailored support and resources to ensure that patients receive the care they need to manage their health effectively.
Medication-Assisted Treatment (MAT): In San Mateo County, we provide Medication-Assisted Treatment (MAT) services. MAT is an evidence-based approach to treating substance use disorders. It combines medication with counseling and behavioral therapies to support recovery from addiction.
Our Healthcare for the Homeless (HCH) program in San Mateo County is dedicated to serving the unique healthcare needs of individuals experiencing homelessness. We provide a range of medical and social services to address the challenges faced by this vulnerable population.
