Referrals
Specialty medical services are available to patients by referral.
Ravenswood Family Health Network (RFHN) ensures seamless patient care by offering continuity of services and making referrals for specialized care as needed. The organization establishes clear guidelines for healthcare providers to initiate and review referrals promptly, incorporating them into the patient’s treatment plan. Additionally, RFHN outlines procedures for referral staff and the provider team to transmit completed referrals, including essential clinical and patient information, to the specialty clinic. We also coordinate the patient’s initial and follow-up appointments, obtaining and delivering relevant consult notes to the provider for timely review.
Our referrals team provides the following services:
- Home health visits
In certain situations, patients may require healthcare services in the comfort of their own homes. Our Referrals Department can arrange home health visits by provider request. This service is ideal for patients who are unable to travel to a clinic due to medical conditions or circumstances
- Transportation to Appointments (Available by Provider Request)
We understand that attending medical appointments can be a challenge for some patients due to mobility issues or lack of transportation. At your healthcare provider’s request, our team can coordinate transportation to and from medical appointments, ensuring you receive the care you need.
- Process referrals to specialty clinics (out patient care, based on patient coverage)
Providers using standard clinical guidelines and local specialist pre-referral recommendations will request a referral when indicated after doing the appropriate initial care and will task it to the referrals staff or care team. After the consult note is received they will review and incorporate and document relevant recommendations into the patient care plan and electronic health record.
- Authorization requests
For both new and established patients, our Referrals Department assists in obtaining necessary authorizations for specialized treatments or medical procedures. We work closely with your healthcare provider’s to ensure all the required paperwork and documentation are submitted accurately and on time.
